Medical Records Request

For law offices and third parties requesting medical and billing records on behalf of BillWell client practices.

HOW IT WORKS

Four steps from request to records

1

Complete the request form

Submit the form below with a valid HIPAA authorization, subpoena, or court order attached. The form will not submit without the attachment.

2

Receive your MRR number

An encrypted acknowledgment arrives by email with your ticket number (MRR-####). All correspondence for this request happens on that email thread.

3

Verification and processing

We verify the authorization and prepare the records. Requests are typically completed within 7 business days of a complete submission, and subpoena return dates are honored.

4

Encrypted delivery

Records are delivered by encrypted email on your MRR thread. There is no charge for records requests.

What we need to release records

BillWell releases records on behalf of its provider clients. A valid HIPAA authorization, subpoena, or court order is required and is verified before any records are sent. Incomplete or invalid authorizations are the most common cause of delay—if anything is missing, we’ll tell you exactly what’s needed on your MRR email thread.

ALREADY SUBMITTED?

Following up on a request

Reply to the encrypted email for your request—the subject line contains your MRR number. Anyone at your firm can reply on that thread, even if they didn’t submit the original request. Please include the MRR number in any correspondence; there’s no need to call.

STEP ONE

Complete this form

Have your authorization document ready to attach. You’ll receive an encrypted acknowledgment with your MRR number after submitting.
